It is advisable to have your blood pressure check at least once a year if you are 45 or over. Please ask about this next time you see your doctor.

If your name has changed or you have moved house but still within the practice catchment area, or if there is any change in your personal details, you need to notify the practice to update your records by filling in a form which can be obtained at the reception.
