New patients registration
To register with the Practice, you must live within its catchment area.
When you register with the Practice, you will be given a registration form and a simple health questionnaire to fill in and then you will be asked to go to the Treatment Room for the nurse to check your blood pressure and other health status. You will also be asked to provide a urine sample for a simple test.
You may express your preference for a particular GP in the Practice at registration.
Please note that in the event of violent or abuse behaviour by a patient to doctors, nurses, any member of staff or any person present on Practice premises, we have the right to remove the patient from the premises. If necessary the Police may be involved and we may request immediate removal of the patient from the Practice list.
Change of Details
You need to fill in a Change of Details form and hand it back to the reception.
The form can be obtained at the reception.

Disabled patients
The Health Centre provides full access for disabled persons.
Wheelchairs are also available to use on the premises.
There is also designated parking space for the disabled in the Health Centre car park.
Blood tests
We offer facilities to have blood tests done on the premises.
It is performed by our trained phlebotomist.
A walk in service is offered on Monday and Friday from 8:30am to 1:00pm.
However, it is BY APPOINTMENT ONLY on Tuesday, Wednesday and Thursday.

Test results
If you have had any test done recently, it is your responsibility to obtain results.
Results can be obtained by phoning the reception after 4:00pm.
Generally, blood results are available after 1 week and X-ray result after 2 weeks.
For confidentiality reason, results are only released to the patient involved, unless there has been prior written consent.
Travel advice

Immunisations
We offer various immunisations including Flu vaccine, Pneumonia vaccine, Tetanus, Diphtheria, Polio and other childhood immunisation.

Private patients
We offer private patient service to visitors from other countries who need medical attention or advice.
Please check with the reception for the current fee for this service.
Please note that this service is not available to patients on NHS list.
Confidentiality
We respect your right to privacy and keep all your health information confidential and secure.
All our staff are bound by the Data Protection Act of 1990.
You are entitled to have access to your written and computerised health records, subject to the limitations of the law.
We will always ensure that we have your written consent prior to releasing any information about you to parties outside the NHS.

Overseas patients
If you are a patient from outside the UK (including Europe) you are entitled to emergency or 'immediate necessary' health care from the NHS. Examples of health problems might be severe chest pain or serious infection.
However, we do not accept overseas patients as NHS patients for routine matters.
If you are from a European country or one with an agreement with the UK, you will be entitled to NHS treatment but individual GP's or practices are not obliged to offer this. We have chosen not to do so.
You may approach other local surgeries as they may have different policy and accept you as an NHS patient.
You may also be charged for hospital treatment even if you receive NHS care from a GP unless you are 'ordinarily resident' in the UK.

Sickness Certificate
You do not need a sick note from the doctor if you are off ill for less than seven calendar days. In this case you will need to fill in a SC2 self certificate which can be obtained from your employer.
Non-NHS services
Some services are not provided by the NHS and there will be a fee for these services.
Examples of these services are private sick note, private prescription, holiday cancellation form, fitness to travel certificate, HGV/PSV medical examination...

Access to medical records
The practice is registered and complies with the Data Protection Act 1998.
We adhere strictly to this Act when dealing with a request to access medical records.
